Admission Policy
  1. Meadowview Manor will admit only those patients whose needs can be met with its services or in cooperation with its contracted services to ensure quality
    patient care.
    • The following conditions are not within the scope of services of Meadowview Manor. (This list is not all inclusive)
      1. Mental illness requiring in-patient care/specialized services.
      2. Communicable diseases including active TB.
      3. Individuals with infections or positive cultures for which appropriate room accommodation cannot be made.
      4. Currently diagnosed alcoholics or drug addicts
      5. Behaviors that are harmful to self or others
      6. Tracheostomy care
      7. NG Tube
  2. Physician recommendation for admission is required.
  3. An approved PAS-2000 dated within 60 days of admission is required.
  4. The following tests are required. (Acceptable if completed within past 30 days)
    • CBC
    • Urinalysis
    • Chest X-ray
  5. Applicants (when necessary) should have a health care decision maker appointed or a medical power of attorney or court appointed committee.
  6. Medicare, Medicaid, and VA benefits are accepted.
    • All applicable federal and state criteria must be met for the appropriate entitlement program.

Applying for Admission

Admission to Meadowview Manor
Thank you for selecting Meadowview Manor as a possible choice for placement of your loved one. The following steps should be taken to apply for admittance.
  1. Complete an Application for Admission.
  2. Provide recent medical information.
    • If the applicant is currently hospitalized request the hospital caseworker to provide this information to the Facility.
    • If the applicant was recently discharged from the hospital provide the Facility with the transcript of the hospital record including admitting history, physical and discharge summary.
    • If the applicant resides at home provide information from recent physician visit.
      1. Diagnoses
      2. CBC, Urinalysis & Chest X-ray. (Acceptable if completed within past 30 days)
      3. Assessment of mental status
      4. Current medications and treatments
      5. Diet
      6. C & S of any open wounds, decubitus ulcers, etc.
  3. Complete a PAS (Pre-Admission Screening) form. The State of West Virginia requires that a physician complete this medical assessment for all applicants of long term care facilities. The assessment is required to determine medical eligibility. If the applicant is currently hospitalized, this form should be completed at the hospital, otherwise it can be completed by the applicant's personal physician at the physician's office.
  4. Submit a copy of the MPOA/Health Car Surrogate/Court Appointed Committee and FPOA documents if applicable.
  5. Submit a copy of the applicant's Living Will, if applicable.
  6. Meadowview Manor contracts with the following physicians to provide services to the residents of the Facility. Applicants' personal physicians are welcome to provide services at the facility by contracting with the Facility. Additional information can be obtained by contacting the Administrator.

Paying For Service


Meadowview Manor participates in the Medicare program and therefore accepts payment from Medicare for individuals who meet the eligibility requirements for Part A benefits and meet the medical eligibility requirements for skilled services.

Eligibility for Part A Benefits

  • Individuals age 65 or older
  • Individuals under age 65 with disabilities
  • Individuals with End Stage Renal Disease (ESRD)
  • Individuals with Amyotrophic Lateral Sclerosis (ALS/Lou Gehrig's disease)

Eligibility Requirements for a skilled nursing stay.

  • A 3 day qualifying inpatient hospital stay (Day of discharge is not counted as an inpatient day).
  • Admission to a skilled nursing facility within 30 days of the hospital discharge.
  • Require a skilled level of care on a daily basis.

Medicare Part A helps cover the cost of skilled nursing facility care.

  • Covers the cost of care for the first 20 days of a benefit period.*
  • Covers a portion of the cost for days 21-100. A co-insurance amount of $144.50 per day for 2013 is the responsibility of the beneficiary either through private pay, Medicaid, or supplemental insurance.
Note: Medicare will help cover the cost of care for up to 100 days provided the patient is receiving a qualified skilled level of care on a daily basis. Medicare benefits cease when the patient falls below a skilled level of care whether or not the full 100 days have been used.


Meadowview Manor participates in the Medicaid program. Medicaid is a State Program available for individuals who meet financial and medical eligibility requirements.

Eligibility for Nursing Home Medicaid

  • Asset level for individuals is $2000 or less.
    - Application must be made and submitted to the local DHHR office.
  • Applicant must need direct nursing care 24 hours a day, 7 days a week.
    - An assessment tool, The Pre-Admission Screening Tool (PAS-2000), must be completed to determine the applicants need.

Private Insurance

Private Pay

Individuals who are not eligible for Medicare, Medicaid or who do not have private insurance would be considered Private Pay. Contact our Admissions coordinator for current rates.
